Abstract

Introduction: Research shows that most seclusion events in pediatric psychiatric settings involve pediatric patients with neurodevelopmental disorders, particularly autism spectrum disorder (ASD). A lack of provider expertise contributes to elevated seclusion rates, highlighting the need for evidence-based, ASD-informed care.
Purpose: To reduce seclusion events within a pediatric psychiatric facility in South Texas, by implementing an evidence-based intervention upon admission that could decrease aggressive behaviors and the likelihood of requiring seclusion. Evidence summary: Literature and guidelines underscore the importance of identifying sensory triggers and calming strategies at admission. Collaborative care and personalized interventions, such as a patient information sheet that identifies patient preferences and behavioral triggers, have been shown to reduce seclusion events in pediatric patients with ASD.
Project goal/objectives/interventions/methods: Seclusion events will decrease by 25%. All clinical staff will be trained on the ASD information sheet, completed by caregivers and filed in the patient binder. The receiving RN will review, sign, and brief mental health staff on key details. Results: April 2025: 3-12 yrs: 0.35 (below), 13-17 yrs: 0.20 (equal), May 2025: 3-12 yrs: 0.38 (below), 13-17 yrs: 0.41 (above), June 2025: 3-12 yrs: 0.35 (below), 13-17 yrs: 0.14 (below)
Implications: The implementation of an evidence-based intervention—initiated at the point of admission—demonstrated effectiveness in supporting clinicians with de-escalation strategies for patients with ASD that decreased seclusion rates facility-wide. When clinicians are informed about a child’s triggers, the care team is better equipped to proactively manage behaviors, thereby minimizing the likelihood of aggression and the need for seclusion.
